The case you posted is a clear presentation of long duration, repeat exposure to nickel which is commonly seen in a medical practice.       So true. I have found that the match between the appropriate mouthpiece shank and leadpipe bore can make or break the sound generated by a horn. This takes me back in memory to a lesson I had with Claudio Roditi, when he had me try out a horn he bought for $75 at a street sale in Upper Manhattan. I played the horn with my mouthpiece and I said, "Claudio, it sounds like a $75 horn". Claudio then gave me a mouthpiece hand crafted for that horn Jerome Callet and the horn just sang. At the end of that lesson, Claudio took me to Jerome's studio where he crafted me a perfect mouth piece for my Olds Recording. All Jerome did was go back and forth on his lathe at grinding on the backbore.

    • Can Playing with Musicians Out of Tune Give You a Headache?

      I received an email question from a musician that I would like to share with this group. Here is the context and the question:

      "[A] guitar player was grossly out of tune. The way I was positioned on the stage allowed one of my ears to be pointed toward a monitor and the other ear pointed toward the audience... The sensation in the ear that was pointed toward the monitor "felt different" than the ear that was pointing toward the audience. This makes me wonder if there is more of a biological/acoustical component to the headaches people get from listening to out of tune performances and its not just tension headaches. Could it be that the cilia react in a way that displeasing when exposed to frequencies that are dissonant? Is it the way the frequencies hit the eardrum?"

      I have never experience such a complaint in my medical practice so I did a PubMed search and found no articles specific to musical intonation and headaches; however, I am sure band conductors can be driven to headaches though when members play out of tune during a performance.

      With that said, I actually did find an article that did note that hearing differences related to tinnitus or sensorineural hearing impairment (difference between hearing quality of ears) is linked to headaches with a 12.2% increase in headaches in patients with this disorder compared to 5.5% in patients with headache without this disorder. I am providing the link below to that abstract. So it just may be possible that some individuals may be prone to headaches by a neurological trigger stimulated by listening to out of tune performers within an ensemble.

      My request to readers of this thread is to please do let me know if any of you have experienced this effect.

      @ROWUK your cautionary comment is well understood. My work out is prioritized for increasing chest volume (capacity) rather than strength as I agree that brut force is our enemy.

      As for the abs exercise, that's more for toning my middle age spread. 🤗

      In addition, I refer you to this article that gives a very good differential diagnosis of lip dysfunctions in which Dystonia, thankfully is not as prevalent as other treatable etiologies.

      Isn't were to breathe a part of musical expression? If the breath marks are not inserted (and there are no inferred vocal lines in the chart) isn't it fair game to breathe as the artist performing would desire? We too are artists and can paint with or breaths and phrasing.

      To me, a jazz musician, breathing and phrasing is how we express our voice. Now when it comes to a jazz, contemporary, pop, gospel piece, the words behind the notes should dictate the breathing and phrasing. But the artist still has their prerogative to rephrase the sentence toward a fresh presentation.

      Interesting perspective that "my horn" is really someone else's horn that the listener tends to admire. That was what roped me into the Getzen 4-valve Etena. Was a real fan of Rick Braun, and at the time I was in the market for a flugelhorn, that is the horn I chose and he is the reason why. However, I sound like me on it, not like Rick Braun, and I am OK with that.

      I use to teach trumpet lessons (Buddy Rodgers Music, North College Hills) when I was a starving college student trying to make extra cash as a musician. That and my band paid my way through college. And I still perform today, 50 years later as a musician (and a card carrying AFM member to boot) but no longer teach trumpet. I now teach however, in my other chosen profession, health care. Wish I had the wisdom way back then, 50 years ago, that I have now as I would have been much more accomplished as a trumpet teacher.

      Fast forward to today. I address my patients by putting them into the prospective that you (the patient) are my boss, and I am only your spiritual advisor. I then let them know it is my role in our relationship to provide them with the relevant clinical expertise centering around their needs, and then provide them several options. They choose the option they want (or sometimes even create an option I did not even bring forward to them). I will go with whatever direction they provide (as long as it does not harm them - "first do no harm"), but then follow them up shortly to take inventory as to where they are and if their goals are being achieved. If so, continue in the same direction, if not, try one of the other methods that I provided based on well documented evidence.

      Key is, you got to get their trust to move forward and if you hear and work with each other, a successful outcome will be achieved.
