What is this mouthpiece?

I was just able to try out this mouthpiece from my lessons teacher and neither him or I have any idea where it is from, how old it is or what model it is. Do any of you have any idea of the model or if / where I can purchase one? Thank you all.

This mouthpiece appears to have been manufactured decades ago. Without any markings on it, we haven't much clue as to who made it. The only thing similar today is by Best Brass http://www.bestbrass.com/mouthpiece/ . The mouthpiece in your picture appears to be modular, whereby the cup section and the backbore/shank section are interchangeable with other internal shapes to tailor a mouthpiece to a particular player's preference.
